2007 Grant Schedule and Proposal Format

Applications for funding must be postmarked between June 22, 2007 and August 24, 2007.  No applications will be accepted if they are postmarked after the August 24 Deadline.  Grants will be awarded and announced in the second week of November and funded in early December.  Please do not call for a grant status report.  You will be notified if additional information is needed.  Grant Guidelines, Cover Form, and this Checklist have been revised for 2007.  Additional copies may be downloaded from this website.  Do not use forms from a previous year. 

APPLICATIONS FOR GRANTS MUST INCLUDE:

The information should be placed in the order shown.  If an item is omitted, enclose a statement giving the reason.

1. Stamped, self-addressed postcard (if you wish to have confirmation of receipt by the Foundation) OPTIONAL

2. Grant Cover Form -  Be sure to provide a complete, concise description of your project.

3. Cover letter or narrative - The letter or narrative should provide information on the history of your organization and additional information on your project.

4. Project budget

5. List of Board of Directors WITH CURRENT HOME ADDRESSES

6. Summary of contributions by Board of Directors - Show the total contributions and the percentage of Board members who made contributions during the last full fiscal year.  Do not itemize individual contributions.

7. Latest IRS Form 990 Tax Return -  If your organization is not required to file Form 990, enclose a statement to that effect.  All financial information submitted should be for your organization only.  If you are part of a large   national organization, such the Salvation Army, remember that we need financial information on the local organization only.

8. Balance Sheet and Statement of Income & Expense for most recently-completed fiscal year and interim statements dated no earlier than March 31, 2007.

9. Most recent Audited Financial Statement - If your organization has not been audited, enclose a statement to that effect.

10. Evidence of income tax exemption from the IRS - Provide a copy of your IRS 501(c)(3) designation or other proof of charitable status.

11. List of other organizations to which you have applied for funding and status of applications

12. Other pertinent information you may wish to include - You may include brochures, news clippings, etc., if you wish.

APPLICATION DEADLINE:

Applications must be received or postmarked by August 24, 2007.
